What is home health care?

Home health care covers a wide assortment of in-home medical services. With the exception of emergent or surgical care, almost any care that can be performed in a hospital can be offered at home. Of course, most patients prefer remaining at home. At Harmony, we’re committed to help you maintain your independence.

Our qualified teams of Registered Nurses, Certified Nurse assistants, and therapists help patients heal with speed and comfort in the privacy of their own home. In addition to coordinating prescribed care with the patient’s physician, we provide progress updates and adjust care according to the doctor’s instructions. Harmony staff provide curative care and teach patients about disease processes and medication administration—a simple service that makes a significant difference in patients’ health once they are discharged from home care.

Who qualifies for home health care?

Anyone who has recently experienced a change in medication or health condition, had a recent or past hospitalization, and currently experiences difficulty leaving home may qualify for home care. Common reasons for home care include:
•    Wound care
•    Diabetes
•    Physical therapy
•    Any disease process that is—or should be—managed by a physician
•    Medication Education
•    Requiring professional help in understanding and managing personal health care.
Our care teams offer the tools that will help you or your loved one regain and retain optimal health. And since health care instruction is one of our highest priorities, our specialists will be happy to answer any questions you might have concerning your health and comfort.

When is home health care appropriate?

You don’t have to wait for a physician to recommend home health care. Anytime it becomes more comfortable and convenient to receive care at home or in a care facility, home health care may be an option. Most patients find home care a more pleasant and satisfying way to heal. Simply ask your doctor if it would be appropriate for you. Please call us at 1-877-I NEED CARE if you have any questions about qualifying for home health care.

Can I choose which agency provides my care?

Yes. By law, all doctors and hospitals are required to allow you to choose which home care agency you use.

What kinds of home health care services are available?

Harmony provides a compassionate, experienced, and highly qualified staff to care for the needs of you or your loved one. Some home services include:
•    Nursing
•    Physical, occupational, and speech therapy
•    Certified Nurse Aids
•    Homemaking services
•    Wound care therapy
•    Private duty
For eligibility requirements and specific information on health care services, please call us anytime at 1-877-I NEED CARE.

How long does it take to receive home health care?

Once you contact Harmony, we immediately contact your doctor for approval and instructions. If you are referring a loved one, we will also contact him or her. After we obtain approval, home care can begin in less than 24 hours. To begin the admission process, simply call 1-877-I NEED CARE. We’re available 24 hours a day, seven days a week.

How often will a home health aid visit the patient?

As often as necessary. At Harmony, a patient’s well being is always more important than our bottom line.

Will the same nurse or therapist visit regularly, or will it be someone different every time?

Consistency is essential in providing safe and effective health care. Once we determine your needs and assign a nurse or therapist, they will continue as your home care provider.

Who administers medication?

Our highly qualified Registered Nurses administer injectable and IV medications. Patients and caregivers can receive education on administering eye drops and medications taken orally.

Is there a co-pay?

There is no co-pay with Medicare and VA. Private insurance plans vary, but often cover all your needs. Harmony will help you identify your plan’s benefits.

Who should I call if I have a question about health care needs, treatments, or coverage?

Please call us day or night at 1-877-I NEED CARE. If it is an immediate medical emergency, call 911.

Testimonials

“We switched to Harmony because a previous agency couldn’t keep the same staff coming. A friend who is a professional caregiver had used Harmony and recommended them to us. We loved our Harmony nurse, Stacy. She was a gift from God, and Maria was a piece of Heaven. I don’t know what we would have done without them.”
– Yvette, Caregiver

“Your service was perfect, dignified. The aides were wonderful, they went above and beyond. You made the process beautiful and I hope I will have the same opportunity when my time comes.”
– Joyce G, daughter of Harmony patient

“I put all my faith in Harmony and you never let me down.”
– Afton H, wife of Harmony patient

“This was so much better than in the hospital. Your staff came through the worst snowstorm, the worst day of the year. I don't know how they did it, but they made it.”
— Lavelle L, wife of Harmony patient

"My therapist is incredible!  He always arrives on time and is a big help to me as I learn how to walk again.  Thank you Harmony."
– Sally

"The service at Harmony is amazing.  We have never experienced such a caring, professional, and competent group of people.  They have really helped us."
– Bill

"Harmony was there for us when we did not  know where to turn.  My wife was getting out of the hospital  and I had no idea how I was going to manage her care all by myself.  Harmony provided all of the care necessary to get her back on her feet."
– George

"Thank you Harmony.  As my mother was approaching the end of her life, I felt like the nurses and chaplains at your company went the extra mile when visiting us.  They added an unexpected level of service that made all the difference."
– John

"I love working at Harmony. The most satisfying part of my job as a nurse is going into the home of a patient and improving their quality of life. Our patients are our number one priority."
– Mary
