Home health care or hospice care services may require a physician’s referral, depending on the method of payment. The professional care team at Harmony Home Health & Hospice is on hand to help determine if a referral is needed, and to walk you through the process of obtaining a referral if necessary.
Once the referral has been received, we can immediately begin the admission process for either home health or hospice services. We will schedule a time to meet with you and your loved one in need of care, right in the comfort of home, and free of charge. During this time, we’ll answer any questions, and gather information to create a personalized plan of care to ensure all needs are addressed. We will also be in communication with the physician to ensure everyone agrees with the treatment plan, and then begin services – usually within 24 hours.
